Step 4: How do I register?
To register as an ARD, fill out the correct application form for your type of business and submit it to the PO Box on the application, along with the application fee.
Getting the correct form:
There are 3 ways you can get the application form:

Download the application online:
- Limited Liability Companies (LLC's) use this application form
- Other types of companies (Sole Proprietorship, Partnership, Corporation) use this application form
- Visit a local field office to pick up an application.
Warning: The application must be submitted to the Department of Consumer Affairs (DCA) at the address listed on the application. You may fill in the application on your computer, but an original signature and date are required. It is your responsibility to ensure that you complete the entire application and submit it with the correct fees. Submission of an incomplete or incorrect application will result in the delay of processing your application.
